As far as ive seen the death feature doesnt work. So i wouldnt worry about it yet. Should be able to enable it in the campaign settings if you hit "Esc". I'm pretty sure the feature works. Death doesnt work, i've enabled it and the character just goes wounded instead of actually dying.

My medicine skill has been increasing automatically during battles as soldiers get wounded. I think the skill also implies if you wait in a town when you have wounded soldiers it will also increase.
